Registration Instructions

  1. Complete the form above.
  2. Remember to check whether you are a TISQA Member in year 2004.
  3. Click the submit button.
  4. You will now be asked to click on the PayPay button to pay by credit card.
  5. Follow the PayPal instructions.
  6. If you choose to not pay by credit card using PayPal you may cancel before making payment and your credit card will not be charged.
